Perinatal Substance Misuse - Screening Tools

SBIRT Oregon provides a screening app and other materials for providers to address substance misuse with perinatal patients.

Access the Behavioral Health Risks Screening Tool for pregnant women, 5Ps: Parents, Peers, Partner, Past and Present for use in your practices.  The tool consists of eight questions about tobacco, alcohol, and drug use as well as questions about depression and intimate partner violence.

The National Institute on Drug Abuse (NIDA) provides the Tobacco, Alcohol, Prescription medication screening tool (TAPS)  that can be self-administered or administered by a clinician.

The Institute for Health Recovery provides information and samples for providers to develop a Safe Care Plan for their patients.

Access a fact sheet for patients with information on how methadone and buprenorphine impact childbirth, breastfeeding and infant care.

The Substance Abuse and Mental Health Services Administration (SAMSHA) maintains a confidential and anonymous searchable database for patients seeking substance abuse treatment and other mental health services.

SAMSHA also provides a confidential, free 24-hour, 365 day a year helpline for individuals and family members facing mental and/or substance use disorders. Patients can also call at 1-800-487-4889.

The Illinois Helpline provides assistance to patients or their family members regarding substance abuse issues and assistance with locating resources.  The Helpline is the only statewide, public resource for finding substance use treatment and recovery services in Illinois. The helpline provides assistance to people using opioids and other substances, with or without insurance.  Assistance is available through their website or by calling 833-2FINDHELP.  Assistance can also be provided in Spanish.
